1. What does C4L do?
C4L arranges for you to receive professional
medical services giving you privacy and convenience. These
services include a review of your medical questionnaire,
your medical records and a FACE TO FACE medical consultation
to determine which, if any treatment and/or medications
may be suitable and proper for you.
A prescription for such medications
may then be issued for you to fill at the pharmacy of your
choice.
2. What is the cost for these services?
The charge for the review of your
medical records and the face to face consultation is $280.00;
the charge for subsequent quarterly telephone consultations
is $180.00. For these subsequent scripts, you also pay for
the delivery charges for the script according to the price
lists.

for you to choose from (see payment alternatives).
3. Why are medical records required?
These records assist the medical
professionals in evaluating your condition, and along with
the consultation, enable them to assist in helping you.
Also they substantiate your medical condition.
This medical process is not diagnostic
and is not designed to evaluate the condition of your health
from the statement of your symptoms; that’s what your
regular doctors are for.
4. How do I find out how I can be helped?
During your face to face consultation you may discuss your
situation with the medical professional and have your questions
answered, including the appropriateness of various drugs.
At the end of your consultation you
can find out which treatments, including medications, are
being prescribed for your use.
5. How do I begin the medical process?
Simply click on “NEW MEMBERS”
for full instructions.
You will be required to complete
the registration, fill out the medical questionnaire provided,
submit your medical records via fax (fax 607-697-0890) as
instructed including a government issued photo ID showing
your birth date, review the TERMS AND CONDITIONS and the
PRIVACY STATEMENT and acknowledge your review and acceptance.
If your medical information is satisfactory, at that time
payment is required and the consultation can be scheduled.
6. What is the timing?
Your consultation can usually be
scheduled after your documentation has been received at
a doctor’s location and time convenient for you.
If a prescription for medications(s)
is considered appropriate for you, the prescription will
be given to you by the doctor after the face to face consultation,
or delivered to you after the 3-month telephone follow-up
consultation.
Consultations generally are not scheduled
on Sundays or holidays, and script deliveries are not made
on Sundays (Saturday script delivery is available to many
areas for an additional charge)
7. How long is my prescription good
for?
Upon your face to face consultation,
the team of Dr’s & PA’s will write your
initial prescription for up to 2 refills. A refill my not
be requested until 25 days after the previous medications
have been received. After the initial prescription and two
refills have been received your medical history must be
updated including a new quarterly consultation by telephone.
8. What about privacy?
All the information provided to us
is treated securely and is viewed by the medical professionals
and C4L administrative personnel only on a need-to-know
basis and is not available for any purpose outside our system
(except, of course, for disclosures required by law). You
agree that the results of your medical consultations may
provided to C4L for your benefit and to complete your medical
records.
9. Is C4L process legal?
C4L arranges medical services within
all the established federal, state and local laws and regulations
and uses only U.S. licensed medical professionals. Restrictions
are placed on various services in certain states and medical
services are not available where they are illegal or prohibited
by federal, state or local laws and regulations.
